Hello! My name is Lawrence Jones of Jones & Associates. I recently received your email. I filled out the form. I had no problem until I got to payment choices. I did what you told me . It still wanted a credit card information.

You selected "credit card" as a payment instead of "check." Not a problem, just ignore that page.
